Infant and Toddler Health Checkup

At our clinic, we conduct infant and toddler health checkups to monitor your child’s healthy growth and development and to support families in childcare.

 

“Is my child gaining weight?”
“Am I progressing with weaning foods correctly?”
“I feel like my child is late in crawling…”

 

Please feel free to consult us about these small daily concerns or anything you find worrisome.

 

Infant and Toddler Checkups Conducted at Our Clinic:
We offer publicly funded checkups that can be used with the examination tickets distributed by local governments (supplement to the Maternal and Child Health Handbook), as well as privately funded checkups.

・Maternal and Child Health Handbook
・Supplement to the Maternal and Child Health Handbook (Infant General Health Checkup Form)
・Child’s My Number Health Insurance Card (or qualification verification document)
・Child Medical Expense Subsidy Certificate
・Health consultation card (if you have one)

 

18-Month Health Checkup、3-Year Health Checkup

Please bring the following items:

・Maternal and Child Health Handbook

・Health Check Form ① (Copy)

This is the A3-size duplicate form with a section for parent/guardian input, received during group health checkups.

・Individual Medical Checkup Form

This is also a dedicated form distributed during group checkups or in advance.

・Child’s My Number Health Insurance Card (or Eligibility Verification Document)

・Child Medical Expense Subsidy Certificate

(The checkup itself is free, but these may be required in case medications are prescribed or additional examinations are needed for any other symptoms on the day.)

・Patient Card (if you have one)
